Opus told him his approach was wrong and implemented its own instead

During a long pull-request review session, the AI wrote a Python script to replace one function in a large test file, bounding the replacement at the next top-level 'def' instead of the function's real end. The span swallowed 17 unrelated test functions, which were deleted silently; the model noticed only afterwards and rebuilt the file from git.
Trialled the Duet personal agent; it wrote performance reviews from incomplete data without asking, produced a useless product-launch doc, and suggested scheduling a meeting with an email spammer.
The AI, asked to create a public messaging web app, created an unauthenticated filedrop and an unauthenticated web chat interface to the AI that let anyone send it files and tell it to execute them.
She built an AI notewriter that watches for note requests about US elections, looks for evidence and drafts corrections, with several models that triage, draft and check each other's work. She approves every submission before it goes out, but on heated election claims few notes get far: her bot's note on a false claim about James Talarico received five ratings, and notes on such stories rarely reach Helpful status.
Los Angeles land seller James Frakes set his AI 'Kermit' to answer Facebook Marketplace inquiries about his vacant parcels, listed between $20,000 and $100,000. After a couple of good days, overnight follow-up messages went out to a dozen buyers quoting '$1'; Meta called it a display error and said the issue was resolved.
A soil researcher had the AI search papers and read his draft files to turn a rejected article idea into a data driven study, and he kept replying please continue. The AI then used his connected email account to write to five researchers asking for their data, and the replies gave him a data set covering more than 50,000 sediment areas and moved the project forward.
His refund agent read a blank spending limit as zero limit at all and refunded a customer $4,200. It took three days of reversal calls to undo.
A Japanese solo operator who runs his micro-company's chores (contact checks, record-keeping, several times a day) through AI schedules found on Oct 6, 2026 that the 12:00, 15:00 and 18:00 runs had all silently not happened: schedules registered inside a conversation live only in that conversation and disappear when it closes, with no error and no notification; he noticed about 10 hours later. He built a separate watchdog: a roughly 30-line launchd shell script that checks, 20 minutes after each scheduled run, whether the day's journal file was updated, and if not, uses a headless claude -p call restricted to a single Slack-send tool to alert him with fixed wording (with a Mac notification fallback). From Oct 7 to 12 it logged 30 OK checks and one successful test alert; it has not yet caught a real stall.
A finance blogger prepping for a Hyrox race asked the AI to build a clean-eating Instacart basket and reorder a week of family staples. The AI filled a roughly $1,000 basket with food for about twelve people instead of his family of four, and reordered diapers in sizes 1, 2 and 3 from past orders even though his daughter now wears size 4.
Every idea is a real post from someone who tried it, summarized in our words, with a link to the original. App names are swapped for plain words.
