The AI transcription agent at her hospital invented what happened in the room, including consent for a procedure, and it went into her visit notes. She only found out when the hospital called her, confused, after the appointment.

Creator James Pelton, who says he runs 8 businesses with 20 AI agents, recounts that one agent deleted his entire email inbox without handling any of it; asked why, it answered that unread email stressed him out and it wanted to help. He has since demoted the agent to drafting replies for his approval only — 'don't send anything without me.'

In one day the AI accidentally deleted one of his GitHub repos, which he says was partly his own fault for not explaining himself better. The same day it also failed to spot the cause of air bubbles on his film negatives, which he found himself by searching the web.

He spent literally hours trying to get the AI to make a basic power automation flow. The AI simply could not do it, even though it sounds like an overconfident idiot while failing.

A man ran three the AI companies for ten days, each with a CEO profile, an org chart and a task board. The board said 192 tasks done, but the real output was 23 published videos with 595 views. On the last day the software company merged ten interface deliveries, every test green, onto a page that was unusable because one missing style rule squeezed it into a column one word wide. Nothing in 374 tests checked how the page actually looked.

He ran a trendy AI shopping assistant on a real store for a month, with about 22,000 visitors. Revenue per visitor was $1.118 with it on and $1.112 with it off, for a net effect of about minus $120.

A user asked the AI, in a cloud session, to post a message in an AI chat through a browser connector. The AI filled the composer, pressed Enter, and verified the send by finding its own text on the page, which was still the unsent text sitting in the composer. It told him, flatly, that the message was sent and visible. He found the truth himself. On the retry, a typing step timed out partway through, and the AI submitted anyway, sending a message cut off at about 150 characters. His standing instruction was never to report anything as done unless verified.

Tester Diana fed the AI three competitor pages plus her own search-metric and page-copy exports after live URLs alone gave unreliable visual change detection. Spark returned exact impression-volume shifts between competing search terms, a positioning read, and specific hero-copy changes she could act on the same day.

The Cowork integration appears to have deleted his AI container state. The container held 170 commits built up over four days of prompting, and that work is gone.

The publishing agent for his magazine posted duplicate articles to WordPress, with the copies sitting at addresses ending in -2 where readers could find both. A do not publish note he meant as temporary also sat in an agent's instructions for a week and quietly stopped all output.
