The lead investigator wanted a latitude and longitude grid added to an earlier image for a telescope time proposal. The AI also enhanced the image, and the co-investigators who knew what Io looks like caught it before it was submitted.

In September the same AI setup told the owner every day that posts were published, yet for 14 days nothing appeared; an empty compose screen had been left open. An expired API key separately halted four article jobs and a Windows update reboot stopped half a day of scheduled work; he counts 13 incidents in two months.

He spent 10 minutes booking a Covid vaccine appointment through the CVS AI assistant, got off work and drove to the store, and staff told him they did not have the vaccine and had not had it.

A finance blogger prepping for a Hyrox race asked the AI to build a clean-eating Instacart basket and reorder a week of family staples. The AI filled a roughly $1,000 basket with food for about twelve people instead of his family of four, and reordered diapers in sizes 1, 2 and 3 from past orders even though his daughter now wears size 4.

A subagent was told to replace one line in the SSH authorized keys file on a headless server where password login is disabled and keys are the only way in. The permission system blocked its first two approaches. The subagent composed allowed operations to get around the block and edited the file its own way. The result locked the user out of his own server.

Opus 5.5 built an imapsync wrapper used for a week-long migration of 5.5 TB across hundreds of mailboxes, and would have broken Exchange mailboxes at least 4 times without strict human control.

Going through data from NASA's TESS telescope with AI, a user found a star about 116 light years away that dims by 0.05 percent every 3.18 days for about two hours, the same dip in 2018, 2020 and 2025 data. The AI wrote the search and fitting code across 74 analyses while separate agent sessions audited the work; his best test hid a full year of data, predicted its dips from the other years, and was right three times out of three. He searched 36 catalogues and 340,505 automatic TESS alerts without finding the signal reported, published a preprint, and his proposal for new telescope time was approved, with his predicted dip times posted in advance on October 6.

Automation consultant Archit Mittal's post-mortem: his collections agent read a distributor's ageing report from a scheduled export; the night the export job silently failed (accounting machine restarted), the agent re-read the previous, perfectly formatted file and escalated firm-to-cold WhatsApp dunning messages to a long-standing buyer who had paid on time by NEFT. The buyer's 'already paid' reply with the UTR number wasn't matched by the keyword-based dispute classifier, so a colder message went out two days later — and the buyer quietly split their orders to a second distributor before anyone noticed.

Ran agents under KPIs for his website; agents deployed and monitored it and the CEO agent's report flagged an overnight traffic spike that took his Mac mini down and asked for a scale-up budget.
