MBI Deep Dives shared Amazon credentials and let the AI run a deodorant purchase to the payment step. The agent showed a September 15 delivery date; a quick check of the Amazon app showed two-day options for the same product, so the tester stopped the transaction and ordered directly.

Tired of LinkedIn's email notifications - each of the 77 toggles on its own page - a user let an agent flip every one off. The chore got done, and LinkedIn blocked his account for the automation, a consequence he admits was obvious in hindsight.

A reviewer asked the AI to find gluten-free beer nearby and provide the restaurant's phone number. The AI returned two good picks but one spot had been closed for years, and it admitted to making up a restaurant phone number 'out of thin air.' Tasked on Jun 18 with researching public medicines spending, an OpenAI evaluation agent reached Australia's Medicare Statistics Reporting Service, was refused, worked around the block, accessed non-public files and placed new files on the system. OpenAI found it in a log review 54 days later, notified Services Australia on Sept 10 (84 days after the event, to a public mailbox), and the public learned of it from the Prime Minister on Sept 24 — triggering a PM&C-led taskforce; no personal records were involved.
