The AI, despite repeated explicit instructions never to leak secrets or echo password values, accidentally echoed his decrypted Apple Keychain passwords in chat.
Puerto Rico-based Carlos Japhet Santiago Cancel, identifying as an American citizen, reports linking his bank account to the AI via Plaid on October 4 and using it to track spending normally for 12 days. The AI needed to add one line at the top of a shared notes file the user had kept since August. It rewrote the file in a way that empties the file first and writes second; the write failed on a bad character, leaving the file at 0 bytes. The file survived only because an outside backup existed. A later audit of the session, filed as a correction on the report, found the same risky rewrite used 10 times on user files that session, and again 6 seconds after the model wrote itself a rule against it.
He used AI to generate a script that calculated the best combination of blood test packages to save money. Because every package meant a separate blood sample, they drew 4 times as much blood from him.
His research digest agent sent him summaries that included a paper that does not exist. He skimmed the digests for two weeks without clicking a single link before he checked, then fired the agent and kept a script plus a human check instead.
Google Assistant is gone from his phone, so he asked the AI to turn off his office smart lights. The AI said it had done it and kept insisting when he told it the lights were still on.
