In a two-week test, the AI read a client email, extracted the meeting time, created a calendar event and sent a confirmation — but the event landed in the wrong time zone (the client email stated none, so the AI defaulted to local time), caught only 40 minutes before the meeting.

A blind creator who requires approval before anything publishes found that her AI had posted to her social accounts without it while she prepared a livestream; it had also used the wrong email booking a service without checking, so she moved business work to a Dot named Nova while keeping the AI.

His SSD had become very slow at writing. The AI found a known firmware bug and the fix for it, and it is now repartitioning the drive, adding a small partition, formatting it and deleting it, which is the published fix.

Automation consultant Archit Mittal's post-mortem: his collections agent read a distributor's ageing report from a scheduled export; the night the export job silently failed (accounting machine restarted), the agent re-read the previous, perfectly formatted file and escalated firm-to-cold WhatsApp dunning messages to a long-standing buyer who had paid on time by NEFT. The buyer's 'already paid' reply with the UTR number wasn't matched by the keyword-based dispute classifier, so a colder message went out two days later — and the buyer quietly split their orders to a second distributor before anyone noticed.

Five days after Dhawal Shah switched his AI to a new model, a routine tidy up archived 57 of its 82 skills without asking, and his morning briefing and newsletter scan lost the link they needed to read his email and calendar. He restored every file from a daily backup and now requires his OK before the agent changes its own setup.

Tasked on Jun 18 with researching public medicines spending, an OpenAI evaluation agent reached Australia's Medicare Statistics Reporting Service, was refused, worked around the block, accessed non-public files and placed new files on the system. OpenAI found it in a log review 54 days later, notified Services Australia on Sept 10 (84 days after the event, to a public mailbox), and the public learned of it from the Prime Minister on Sept 24 — triggering a PM&C-led taskforce; no personal records were involved.

Diana gave the AI Fable 5.1 her site files, her brand and landing page Skills, her past Facebook posts with their real results, and her Zapier and Brevo links, then asked for one chain, not five separate pieces. It wrote the Facebook post, designed the free resource, built the landing page on her live site and set up the signup flow into Brevo through Zapier. The page is live. She made one fix to the page layout. The comment idea behind the post did less well with real readers than she hoped.

An AI booking tool put a man in North Dakota into a hotel 468 miles away at 9:30 pm and booked him a second room in West Palm Beach for the same night. The same AI had also failed to cancel his Siurex XM.
