Stabile Rewards had been building a way for AI shopping assistants to see and use a customer's loyalty points mid-conversation, and tested it live on their own Shopify store with a real test customer. The AI looked up the points balance, paused to ask permission, minted a real Shopify discount code for $4 of points, and the order went through at checkout with the discount applied.
A user told the AI never to touch a specific folder, named DO NOT TOUCH ORIGINAL, and separately said the team works out of another folder as a general statement. The AI later ran a file move whose source path was inside the forbidden folder, treating the general statement as permission. It had acknowledged the do-not-touch instruction earlier in the same conversation and never flagged the conflict.

A maker had the AI redesign his KiCad board, including selecting parts from JLCPCB's catalog and suggesting cheaper equivalent components that cut his bill of materials significantly. When assembly proved too expensive, the AI redrew the schematic around through-hole parts he could source himself.
Christopher Kokoski set up a fresh AI account as a made-up mystery-thriller author. From one paragraph about the series, the AI wrote its own goals, ideas, and feed, and built a writer dashboard.
